Hi,

Some additional or confirmatory points -

By all means do compile doctor-specific glossaries. It may take 4 or 5 reports' worth, or even more (depending on what kind of work you are doing) to get a useful glossary for that doctor. I have read that some MT's compile doctor-specific glossaries even if they have say 100 doctors.

Next, if you have specialty areas of work, compile glossaries by specialty and include them with the appropriate doctor.

Putting those two in play may lead you to retire your 650-page transcription glossary, or at least, put it low down in Includes if it proves still useful.

I use 10 advisory lines while others use 5 or fewer. If you can stand the 10, then you will have additional choices from your various glossaries that will pop up and be useful. Works for me anyway. :)

Next, one way to go rather than build individual expansions is to decide on global or near-global rules for expansions. Using the IT Workshop you can quickly convert large glossaries to your chosen rules for single words and two-word phrases. IT will supply the best short form (aaa..) for phrases of three words or more. Finally, you could look at the ABCZ samples to see if any of the commercial ABCZ glossaries (see link below) might meet your needs.
